on a small group plan where premiums of $9,000 to $10,000 a year were needed for
approximately $5,000 in claims.

No. IT-339R2 setting out the meaning of private health services plans (PHSP).
I personally use Cost Efficient Benefit Plan as my provider for Cost Plus services.
